Mysimba (Naltrexone/Bupropion) – Overview

1. Composition & Form

• Each prolonged-release tablet contains:

  • Naltrexone hydrochloride 8 mg

  • Bupropion hydrochloride 90 mg

• Film-coated tablets for oral use.

2. Indications

• Adjunct to reduced-calorie diet and increased physical activity for weight management in:

  • Adults with BMI ≥30 kg/m² (obese)

3. Mechanism of action

• Bupropion: norepinephrine–dopamine reuptake inhibitor (NDRI), reduces appetite and food cravings.

4. Dosage & Administration

  • Week 3: 2 tablets in the morning, 1 in the evening

  • Week 4 onward (maintenance): 2 tablets in the morning, 2 in the evening

5. Side Effects

Very common / common:

• Nausea, constipation, vomiting, diarrhoea

• Headache, dizziness, insomnia, anxiety

• Increased blood pressure and heart rate

Less common but serious:

• Seizures (dose-related risk)

• Suicidal ideation or mood changes

• Severe hypertension

• Hypersensitivity reactions

6. Contraindications

• Uncontrolled hypertension

• Seizure disorders or history of seizures

• Eating disorders (bulimia, anorexia nervosa)

• Chronic opioid use or acute opioid withdrawal

• Abrupt discontinuation of alcohol, benzodiazepines, barbiturates, or antiepileptics

• Severe liver or kidney impairment

• Pregnancy and breastfeeding
